"Reaction [beta]"

Provide us with an email address we should not contact 25 Oct 2007

AT&T Wireless's "Customer Opt-out Request" form asks users to provide an email address that should not be used for the purposes of contacting them. The circled text reads:

"Additionally, if you do not want to receive email communications from us, you must provide us with the email we should not contact."

Amazing! That form looks like a usability nightmare... negative actions on the check boxes (i.e. tick to not receive junk), a lone radio button (is the other one off screen somewhere?), a clear button next to submit and of course the highly bizarre request for an email address not to use. Nice one AT&T.
